Job Summary

We are seeking a highly skilled RN Case Manager to join our team in Home Health. As a RN Case Manager, you will be responsible for coordinating patient care, developing and implementing individualized care plans, and collaborating with healthcare professionals to ensure seamless transitions of care.

Key Responsibilities
  • Conduct comprehensive assessments of patients' medical, functional, and psychosocial needs
  • Develop and implement individualized care plans that address patients' needs and goals
  • Coordinate with healthcare professionals to ensure timely and effective delivery of care
  • Monitor patients' progress and adjust care plans as needed
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless transitions of care
Requirements
  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation
  • One (1) year experience as a RN
  • Licensed to practice as a registered nurse in the state of agency operation
  • Must possess and maintain valid CPR certification while employed in a clinical role
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
