Job Summary

Optimal Home Care is seeking a skilled Medical Social Worker to join our home health care team in the Denver area. As a Medical Social Worker, you will play a vital role in providing social case work and preparing social histories for patients and their families. Your expertise will help patients and their families understand, accept, and adhere to medical regimens, and assist them in utilizing existing resources in the home environment or referring them to community agencies as necessary.

Responsibilities
  • Comply with ethical and professional standards of conduct as set forth by the National Association of Social Workers.
  • Practice and maintain patient safety and dignity by abiding by Federal, State, Local, and Optimal policy and procedures as pertains to patient protected health information and HIPAA.
  • Provide rehabilitative and supportive casework with the purpose of restoring patients to their optimum level of social and health adjustment.
  • Assist patients and their families with personal and environmental difficulties that predispose them to illness and interfere with their obtaining maximum benefits from medical care.
  • Prepare a social history and Plan of Care with the purpose of improving the scope and quality of existing services or extending services.
  • Consult with the Physician and other members of the health team for the purpose of assisting them to understand significant social, emotional, and environmental factors related to the patient's health problems.
Requirements
  • Graduate with a Master's Degree from an accredited school of social work.
  • One (1) year of social work experience in a health care setting.
  • Must be currently licensed and in good standing in the state of Colorado.
  • Must be able to communicate in the language of the client and document client-related activity as appropriate.
  • Possesses adequate and effective written and verbal communication skills.
  • Possess adequate and effective interpersonal skills.
