Job Summary

The Recovery Navigator is a critical role in the WCS Hub and Spoke Health Home, working collaboratively with the team to provide comprehensive care to individuals with significant substance use and co-occurring mental health needs. This position requires a strong focus on engagement, advocacy, and coordination of services to ensure individuals receive quality care and support.

Key Responsibilities
  • Develop and implement individualized comprehensive care plans in collaboration with the person and their care team.
  • Facilitate monthly recovery monitoring and team meetings to review progress and address ongoing needs.
  • Coordinate care with providers of services, verify services are provided, and document all interactions and activities in the electronic medical record.
  • Provide transportation and attend appointments with the person, advocating for their needs and preferences.
  • Support crisis response and develop strategies to meet immediate needs, including developing a comprehensive relapse prevention plan.
  • Network with community providers and Spokes to develop collaborative relationships and meet the needs of individuals served by the program.
Requirements
  • High school or GED/HSED required; Bachelor's degree in Social Work, Psychology, Nursing, or Social Services preferred.
  • Knowledge and experience serving people with substance use needs, including working knowledge of computer applications (Windows and Microsoft Word).
  • Ability to display cultural competence, respond respectfully and effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.
  • Valid driver's license, automobile, and insurance sufficient to meet agency requirements.
