We are seeking a skilled Mental Health Worker II to join our team at SFV Community Mental Health. As a key member of our Coordinated Entry System, you will play a vital role in supporting individuals in their journey towards permanent housing.
Key Responsibilities- Provide direct and indirect client services, including transportation to and from available units and appointments.
- Document services provided in Clarity, adhering to LAHSA guidelines.
- Research and locate available housing units, developing and maintaining professional relationships with property owners and managers.
- Collaborate with fellow housing staff and case managers to ensure a comprehensive approach to client support.
- Complete VI-SPDATs, CES Referral Packets, and COC/S8/EHV applications as assigned by management.
To be successful in this role, you will need to possess excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment. A strong understanding of housing systems and resources is also essential.
