Empowering Youth and Families

This position will engage high-need youth and their families in developing, implementing, and monitoring an intensive, individualized plan of care. The Wraparound Facilitator will support the team as they implement the plan and monitor its success.

Key Responsibilities

  • Coordinate and facilitate monthly Wraparound team meetings to ensure effective collaboration and goal achievement.
  • Provide leadership in the development, implementation, and evaluation of individualized plans using the phases and activities of Wraparound.
  • Implement the YES Waiver program and complete all required documents within identified time frames.
  • Develop trust and mutual respect with youth and their teams, fostering a supportive and inclusive environment.
  • Assist teams to identify needs and create a shared vision, promoting a collaborative approach to care.
  • Assist families in identifying, recruiting, and engaging natural supports, enhancing the youth's support network.
  • Enable teams to take ownership of the Wraparound process, promoting autonomy and self-efficacy.
  • Support clients in identifying their strengths and building upon them, empowering them to achieve their goals.
  • Develop the problem-solving skills and self-efficacy of the youth and team members, promoting a growth mindset.
  • Identify barriers to full participation in the Wraparound process and develop strategies to overcome them, ensuring equitable access to care.
  • Ensure the Wraparound process is completed with high fidelity to the model and track progress towards goals, maintaining a focus on outcomes.
  • Provide support and advocacy for Wraparound teams, promoting a culture of care and compassion.
  • Prepare summary documents and final reports, ensuring accurate and timely communication.
  • Ensure that all required confidentiality and other compliance documents are current for each youth, maintaining a commitment to confidentiality.
  • Educate families, providers, and the community about the Wraparound process, promoting awareness and understanding.
  • Contribute to and support a positive, team-oriented work environment, fostering a culture of collaboration and respect.
  • Utilize Collaborative Documentation and ensure that 95% of all documentation is entered within 24 hours, maintaining a focus on efficiency and effectiveness.
  • Turn Service Request Forms (if applicable) within 24 hours with a denial rate of less than 5%, ensuring timely and accurate service delivery.
  • Provide at least 75% Person-Centered Time each month or an average each quarter, maintaining a focus on person-centered care.
  • Promote and demonstrate Hill Country MHDD Centers' mission, vision, and values through both behavior and job performance on a day-to-day basis, embodying the organization's culture.
  • Effectively interact and communicate with Hill Country MHDD Center staff/persons in service and their families/customers and partnerships, maintaining a commitment to collaboration and respect.
  • Knowledge of or experience with the Wraparound Model is preferred, but not required.
  • Strong written and verbal communication skills are essential, as well as proficiency in computer applications such as Microsoft Word or Excel.
  • Organized and effective multitaskers are highly valued, as are individuals with a passion for connecting individuals and families with excellent care.

Requirements

  • Bachelor's degree from an accredited four-year college or university with a major in social, behavioral, or human services.
  • One year of experience working with children and youth with serious emotional disturbance is required.

Preferred

  • Three years of experience working with children and youth with serious emotional disturbance is preferred.
  • Bilingual (Spanish/English) is a plus.
  • NWIC Model - Wraparound Training is highly valued.
  • A valid Texas State Driver's License and current automobile insurance are required for travel.
