Mission Hospital is seeking a skilled Case Management Registered Nurse to join our team. As a key member of our care management team, you will play a vital role in ensuring high-quality, patient-centered care for our patients.

About the Role

The Case Manager will be responsible for coordinating and directing care specific to the needs of each patient. This includes:

  • Implementing patient treatment plans that support the patient's strengths, abilities, needs, and preferences
  • Facilitating patient involvement throughout the rehabilitation process
  • Documenting findings of Discharge Planning Evaluations and psychosocial assessments
  • Communicating social, financial, or discharge needs and preferences of the patient/family/caregiver
Requirements

To be successful in this role, you will need:

  • Minimum of 3 years in nursing practice
  • Clinical knowledge, IT skills, effective interpersonal communication, critical thinking, successful problem-solving, ability to work autonomously, innovative thinking, resourceful at navigating community resources, team building, and leadership
Preferred Qualifications

Previous experience in nursing practice and care management, community or ambulatory care management experience, primary care clinical experience, experience with inpatient discharge planning work, Motivational Interviewing, and Intrinsic Health Coaching are highly desirable.
