Job SummaryThe Care Manager will be responsible for providing comprehensive assessments, developing care plans, and coordinating care with healthcare providers. You will also provide 24/7 on-call crisis support and transitional care management services.
Responsibilities- Conduct comprehensive assessments to identify member needs and develop individualized care plans
- Coordinate care with healthcare providers, including primary care physicians, specialists, and hospitals
- Provide 24/7 on-call crisis support to members in crisis
- Develop and implement transitional care plans to ensure smooth transitions between care settings
- Monitor member health outcomes and make adjustments to care plans as needed
- Bachelor's degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area
- Two years of experience working directly with individuals with behavioral health conditions
- Preferred experience working with adults and children with mental health and/or substance use disorder needs
