Job Summary

We are seeking a highly motivated and compassionate Family Interventionist to join our team at MHMR Tarrant. As a Family Interventionist, you will play a critical role in providing individualized, trauma-informed services to youth and young adults in home, school, and community-based settings.

Key Responsibilities
  • Assist youth and young adults in transitioning from Child & Family Services to options after high school/GED, encompassing physical and behavioral healthcare, social services, housing, educational systems, and employment opportunities.
  • Act as a liaison for transitioning care and serve as a liaison with MHMR Disability Services.
  • Educate and assist youth in self-advocacy, assess services, negotiate concerns, and navigate support programs.
  • Mentor families for successful transition, providing educational support and advocating for needs.
  • Attend outreach events to educate families on available services, fostering understanding of Disability Services.
Requirements
  • Bachelor's Degree in a Health and Human Services field.
  • Qualified Mental Health Professional (QMHP) certification or eligibility.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively with families from diverse backgrounds.
  • Knowledge of Texas laws regarding reporting adult and child abuse/neglect.
