Job Title: Wellness Nurse

Responsibilities:
  • Identify Resident Needs: Conduct regular assessments to identify the ongoing needs and services of our residents, promoting the highest quality resident services.
  • Ensure Proper Follow-Through: Participate in the evaluation and documentation of residents with changes in healthcare services and status, ensuring seamless transitions.
  • Collaborate with Resident Care Director: Assist in conducting resident evaluations and gathering data at move-in, 30 days post-move-in, semi-annually, and with any change in status.
  • Conduct Monthly Wellness Visits: Regularly visit residents to monitor their health and well-being, documenting any concerns or issues.
  • Monitor and Report Incidents: Document and report any falls, accidents, or incidents, ensuring the safety and well-being of our residents.
  • Communicate with Families: Maintain open communication with residents' families and responsible parties to ensure their medical needs are being met.
  • Collaborate with Attending Physicians: Contact residents' attending physicians as necessary, or upon family request, to ensure comprehensive care.
  • Maintain Wellness Files: Ensure accurate and up-to-date documentation of residents' wellness files, adhering to Sunrise policies and federal/state regulations.
  • Manage Medical Supplies: Maintain medical supplies and emergency kits for the community, ensuring readiness for any situation.
  • Provide Clinical Support: Offer clinical support and assistance to community team members as needed, promoting a culture of teamwork and collaboration.
  • Adhere to Infection Control Practices: Understand and follow infection control practices, partnering with the Resident Care Director and community team to promote infection control standards.
  • Monitor Medication and Treatment Profiles: Regularly review and ensure accurate administration of medications and treatments, documenting any concerns or issues.
Qualifications:
  • Graduate of Approved College/School of Nursing: Possess a degree from an accredited nursing program.
  • Current State License: Maintain a current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Experience in Long-Term Care: Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
  • Knowledge of Nursing Practices: Demonstrate knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
  • Assessment Skills: Possess good assessment skills, with the ability to identify and address resident needs.
  • Communication and Teamwork: Demonstrate excellent written and verbal communication skills, with the ability to teach and guide team members to provide quality clinical care.
  • Organizational and Time Management: Possess competent organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Computer Skills: Demonstrate proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel), and Sunrise applications, with the ability to learn new applications.
