Job Title: Wellness Nurse

Responsibilities:
  • Identify and Meet Resident Needs: Conduct regular assessments to identify the ongoing needs and services of our residents, ensuring the highest quality of care is delivered.
  • Collaborate with Resident Care Director: Work closely with the Resident Care Director to evaluate and document changes in resident health care services and status.
  • Conduct Monthly Wellness Visits: Regularly visit residents to monitor their health and well-being, documenting any concerns or issues.
  • Monitor and Report Incidents: Respond to any falls, accidents, or incidents, documenting and reporting as required.
  • Communicate with Residents and Families: Maintain open communication with residents and their families to ensure their medical needs are being met.
  • Maintain Accurate Records: Document all pertinent information in resident wellness files, ensuring compliance with Sunrise policies and federal and state regulations.
Qualifications:
  • Education and Licensure: Graduate of an approved college or school of nursing, with a current state license as a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Experience: Minimum of one year of experience in a home health, assisted living, hospital, or long-term care environment.
  • Skills and Abilities: Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, as well as federal, state, and local regulations. Ability to handle multiple priorities, possess written and verbal skills for effective communication, and demonstrate good judgment, problem-solving, and decision-making skills.
