Job Summary:
LanceSoft is seeking a skilled LPN/LVN Caregiver to provide high-quality nursing care to patients in their place of residence. As a key member of our healthcare team, you will work under the supervision of a Registered Nurse (RN) to deliver care that meets the unique needs of each patient.
Responsibilities:
- Assist in providing nursing services and identifying patient needs as evidenced by observation, documentation, and clinical record.
- Participate in planning, coordinating, and implementing care to provide quality, cost-effective patient care.
- Provide effective communication to patients, families, and healthcare professionals.
- Assist patients and families in learning self-care techniques and assess their response to care.
- Provide care utilizing infection control measures that protect both staff and patients.
- Assist RN and/or physician in performing specialized duties related to end-of-life care for Hospice patients.
- Assist with admitting, discharging, and transferring patients.
- Possible on-call duties.
Requirements:
- Nursing skills as defined by accepted standards of practice.
- Proof of current Hepatitis consent/declination and TB vaccination record.
- Ability to use electronic medical charting and time-keeping software.
- Reliable transportation and current auto liability insurance coverage.
- Ability to recognize and maintain professional boundaries in relationships with coworkers, patients, and families.
- Ability to recognize an unsafe working environment and notify supervisor immediately.
- Participate in annual in-service and performance improvement programs.
- Must read, write, and speak fluent English.
- Must have good and regular attendance.
Preferred Qualifications:
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
- Current CPR certification.
