RN Case Manager PRN

We are seeking a skilled RN Case Manager PRN to join our team at HCA Florida JFK North Hospital. As a key member of our rehabilitation team, you will play a vital role in ensuring high-quality, patient-centered care for our patients.
Responsibilities- Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.
- Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
- RN with current state licensure; BSN preferred.
- Graduate from an accredited school of nursing required.
- 1-3 years of nursing experience in an acute care setting (i.e., med surg, tele, ICU).
- Previous case management experience within a hospital setting.
- Certification in case management or utilization review preferred.
- Knowledge of InterQual criteria, Medicare, Medicaid Guidelines, and Managed Care.
Per Diem Colleagues agree to be available to work a minimum of five (5) shifts within each four-week schedule as detailed below:
- One (1) of those shifts must be a Weekend shift, OR; the Per Diem Colleague may agree to only work Weekends and commit to work a minimum of four (4) weekend shifts within each four-week Schedule.
- Per Diem Colleagues agree to be available to work at least one (1) shift on one (1) major holiday and (2) minor holidays.
- Per Diem Colleagues agree to be available for full-time orientation/training for 3-5 weeks (varies based on experience).
