We are seeking a highly skilled and compassionate Substance Use Disorder Case Manager to join our team at the Alcohol and Drug Freedom Center. As a key member of our staff, you will play a vital role in providing support to individuals and families struggling with substance use disorders.
Key Responsibilities- Provide case management services to clients, including assessment, goal setting, and service coordination.
- Develop and implement individualized treatment plans that address the client's unique needs and goals.
- Collaborate with community organizations and healthcare providers to ensure seamless continuity of care.
- Transport clients within the county as needed, providing a safe and supportive environment.
- Monitor client progress, making adjustments to treatment plans as necessary.
- High school diploma or equivalent required; CDCA credential strongly preferred.
- Minimum 2 years of experience in human services, social work, or addiction services.
- Strong understanding of alcohol and drug treatment and recovery processes.
- Ability to engage individuals in treatment and recovery, as well as coordinate referrals for ancillary services.
- Excellent interpersonal and communication skills, with a customer-centric approach.
As a valued member of our team, you will enjoy a competitive salary range of $16 to $17 per hour, as well as a comprehensive benefits package, including medical, dental, vision, paid time off, and a 403(b) with employer contribution. The Freedom Center is also a Federal Loan Forgiveness Participant.
