• Provide clinical expertise and guidance to patients, healthcare providers, and pharmacy staff on medication therapy, disease management, and wellness services.
• Conduct thorough patient consultations, including medication reviews, health assessments, and disease state management.
• Develop and implement personalized treatment plans, taking into account patient medical history, medication regimens, and lifestyle factors.
• Collaborate with healthcare providers to ensure seamless care transitions and optimal patient outcomes.
• Stay up-to-date with the latest clinical research, guidelines, and best practices in pharmacy and healthcare.
• Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote patient safety and satisfaction.
• Foster strong relationships with patients, healthcare providers, and community partners to promote health education and awareness.
• Provide immunizations, diagnostic testing, and other healthcare services as needed.
Requirements:
• BS in Pharmacy or Pharmacist Degree from an accredited educational institution.
• Current pharmacist licensure in the states within the district.
• Experience performing prescription dispensing activities that demonstrate a strong working knowledge of applicable state and federal controlled substance laws.
• Certified Immunizer or willing to become an immunizer within 90 days of hire.
• At least 1 year experience as a pharmacist in a retail setting, including prescription filling and verification, records and legal compliance, pharmacy operations, pharmacy software and technology systems, and insurance.
