  • Collaboration and Communication: Work closely with healthcare teams, patients, and families to ensure seamless care coordination and effective communication.
  • Resource Utilization: Identify and connect patients with community resources, services, and support to meet their unique needs.
  • Documentation and Reporting: Maintain accurate and timely documentation of patient interactions, care plans, and outcomes.
  • Quality Improvement: Analyze patient outcomes and program data to identify areas for improvement and implement evidence-based practices.
Requirements:
  • Education: Master's degree in Social Work (MSW) required.
  • Experience: Minimum one year of clinical social work experience in a healthcare setting required.
  • Licensure: Licensed Independent Social Worker (District of Columbia) or Licensed Graduate Social Worker (District of Columbia) within 6 months of hire, and Licensed Master Social Worker (Maryland) and Licensed Master's Social Worker (Virginia) within 6 months of hire.
Preferred Qualifications:
  • Computer Skills: Experience with computer software programs in a Windows environment preferred.
  • Community Knowledge: Knowledge of community systems and resources in the defined service area preferred.
  • Regulatory Knowledge: Knowledge of regulatory issues for the Mid-Atlantic area preferred.
Work Environment:

This is a full-time position with a 40-hour workweek. The schedule includes evening shifts and requires 20% travel time.
