Family Medicine Nurse Practitioner

We are seeking a highly skilled and compassionate Family Medicine Nurse Practitioner to join our team at Franciscan Health Crawfordsville East Side Family Medicine. As a key member of our primary care team, you will provide high-quality patient care, work collaboratively with physicians, and contribute to the growth and development of our practice.
About the Role:- Provide comprehensive primary care services to patients of all ages, including routine check-ups, chronic disease management, and acute care.
- Collaborate with physicians and other healthcare professionals to coordinate patient care and ensure seamless transitions.
- Participate in hospital rounding visits, including history, physicals, discharges, and skilled nursing home visits as needed.
- Develop and maintain strong relationships with patients, families, and community members to promote health and wellness.
- Stay up-to-date with the latest medical research, guidelines, and best practices to provide evidence-based care.
