About the Job:
This is a full-time, remote position with a focus on disease management. The successful candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently in a high-volume environment.
Responsibilities:
- Assess patient needs and develop individualized care plans
- Provide education and training to patients and caregivers
- Maintain accurate records of patient interactions and progress
Requirements:
- Bachelor's degree in Nursing
- At least one active state nurse license, preferably compact state licensure
- Minimum 2 years' experience as an RNDM, preferably in a remote or hybrid environment
- Certification in disease-management related area (e.g., CCP, CHC, CDE, AE-C)
