Admissions and Discharge and Transfer Nurse

The Admissions and Discharge and Transfer Registered Nurse plays a pivotal role in the patient journey by efficiently managing admissions, discharges, and transfers while ensuring seamless transitions and maintaining patient safety and satisfaction.
Key Responsibilities- Conduct thorough assessments of incoming patients, including reviewing medical records, obtaining patient histories, and performing physical examinations to gather essential information for the admission process.
- Collaborate with healthcare providers, social workers, and other team members to develop and implement comprehensive discharge plans that include medication instructions, follow-up appointments, and patient education to promote continuity of care.
- Facilitate the safe and timely transfer of patients between units, departments, or healthcare facilities, ensuring the proper documentation and communication of patient information.
- Act as an advocate for patients and their families, addressing their concerns and needs related to admissions, discharges, and transfers and providing emotional support during transitions.
- Maintain accurate and up-to-date patient records, including admission and discharge documentation, transfer summaries, and progress notes, following established healthcare regulations and facility policies.
- Educate patients and their families on the discharge process, medication management, and self-care instructions, ensuring they are well-prepared for post-discharge care.
- Work closely with nursing staff, physicians, case managers, and other healthcare professionals to ensure smooth transitions of care and effective communication throughout the patient's hospital stay.
- Participate in quality improvement initiatives related to admissions, discharges, and transfers, identifying areas for improvement and implementing best practices to enhance patient care and satisfaction.
- Be prepared to assist during medical emergencies, following established protocols and providing support as needed during critical situations.
- Graduate of an accredited school of nursing; Associate's Degree in Nursing is required.
- One year of experience as a Registered Nurse (RN) is required.
- Strong organizational and communications skills.
- Ability to work effectively in a fast-paced healthcare environment.
- Current license as a Registered Nurse (RN) in the state of North Carolina.
- Basic Life Support (BLS) - BLS from the American Heart Association is required at the time of hire.
Regularly exposed to the risk of blood-borne diseases. Frequently exposed to hazards from electrical/mechanical/power equipment, odorous chemicals and specimens, and unpleasant elements (accident, injury, slippery surfaces, uneven surfaces), etc. Subject to irregular hours (days, evenings, nights, weekends, holidays) and emergency situations.
