AdvisaCare, a leading provider of home health care services, is currently seeking a Full Time or PRN Home Health Physical Therapist to provide quality care in patients' homes.

About the Role

This position involves conducting initial and ongoing patient assessments to determine individualized treatment plans and goals. You will develop and implement physical therapy interventions based on best practice guidelines, patient needs, and physician orders.

Key Responsibilities
  • Conduct thorough patient assessments to create tailored treatment plans.
  • Develop and implement evidence-based physical therapy interventions.
  • Monitor and document patient progress, making adjustments as needed.
  • Educate patients and caregivers on exercises and activities to improve mobility, strength, balance, and coordination.
  • Collaborate with interdisciplinary team members for holistic care delivery.
  • Maintain strict adherence to legal, ethical, and regulatory guidelines.
Requirements

To succeed in this role, you will need:

  • A valid and current Physical Therapist license in Michigan.
  • At least one year of experience as a Physical Therapist, preferably in home health.
  • Strong clinical skills, including assessment, treatment planning, and implementation.
  • The ability to work independently and as part of a team, with excellent communication and collaboration skills.
  • Flexibility and adaptability, with a positive attitude and customer-focused approach.
  • The ability to navigate technology and electronic medical records.
