About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team of healthcare professionals in providing high-quality care to pediatric patients in the comfort of their own homes.

Key Responsibilities
  • Utilize the nursing process to assess, plan, implement, and evaluate patient care, ensuring the highest level of quality and safety.
  • Provide skilled nursing care and services in accordance with prescribed orders, working collaboratively with healthcare teams to achieve optimal patient outcomes.
  • Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition, collecting and analyzing data to inform nursing diagnoses and interventions.
Requirements
  • Current licensure as an LPN/LVN in the state where the nurse will practice.
  • Current TB or Chest X-Ray, as well as a current BLS card.
