As a Nurse Practitioner (NP), you will be an integral part of our clinical operations team, delivering direct patient care across various settings, including medical centers, nursing homes, and home environments. Key Responsibilities

The Nurse Practitioner will:

  • Conduct comprehensive geriatric assessments, including medical histories and physical examinations.
  • Diagnose and treat patients, developing tailored nursing care plans.
  • Provide health education and facilitate physician referrals as necessary.
  • Engage in case management and ensure thorough documentation in line with ChenMed standards.
  • Participate in clinical rounds and contribute to team discussions to enhance patient care.
  • Work collaboratively with healthcare professionals to ensure compliance with guidelines and quality standards.
Essential Skills and Qualifications

The ideal candidate will possess:

  • Current RN licensure and Nurse Practitioner Certification in the relevant state.
  • Certification in Basic Cardiac Life Support.
  • Board certification by AANP or ANCC is preferred.
  • A minimum of one year of clinical experience is preferred, although strong new graduates will be considered.
  • Excellent communication and interpersonal skills, with a focus on patient-centered care.
  • Strong critical thinking and problem-solving abilities.
