Job Summary

We are seeking a skilled and compassionate RN Case Manager to join our team at Novant Health. As a key member of our healthcare team, you will play a vital role in ensuring the seamless transition of patients from hospital to home, while providing exceptional care and support to our patients and their families.

Key Responsibilities
  • Collaborate with physicians and other healthcare professionals to develop and implement comprehensive discharge plans that meet the unique needs of each patient.
  • Conduct thorough assessments of patients' medical and social needs, identifying potential barriers to successful discharge and developing strategies to overcome them.
  • Facilitate communication and coordination between patients, families, healthcare providers, and community resources to ensure a smooth transition of care.
  • Provide education and support to patients and families on self-care, disease management, and community resources.
  • Participate in quality improvement initiatives to enhance the overall quality of care and patient satisfaction.
Requirements
  • Current RN licensure in the state of employment.
  • Associate's degree in nursing or BSN, with a preference for a 4-year degree.
  • Two or more years of healthcare experience, with a preference for case management or related experience.
