Patient Care Specialist

This full-time position offers a competitive salary of $43,500 - $55,000 per year.
**Key Responsibilities:**
* Provide direct and indirect patient care under the guidance of registered nurses and other healthcare professionals.
* Develop strong relationships with patients, families, and interdisciplinary teams to ensure comprehensive care planning and coordination.
* Maintain accurate and detailed records of patient interactions, treatments, and progress.
* Contribute to unit-based quality improvement initiatives and participate in education and training programs to enhance skills and knowledge.
* Collaborate with multidisciplinary teams to address patient needs and concerns.
* Demonstrate a commitment to patient-centered care and strive for excellence in all interactions.
**Requirements:**
* High school diploma or equivalent required; Associate's degree or higher preferred.
* CNA certification required; BSN or relevant experience preferred.
* Excellent communication and interpersonal skills necessary for effective collaboration and patient engagement.
