Key Responsibilities

Requirements
  • Current, unrestricted Social Worker license or LMHC in the state of Hawaii.
  • 2+ years of experience in social work or a related field.
  • Intermediate experience working with MS Word, Excel, and Outlook.
  • Ability to travel in assigned regions to visit patients in their homes and/or other settings.
Preferred Qualifications
  • CCM (Certified Case Manager) certification.
  • Experience with case management and discharge planning.
  • Background in managing populations with complex medical or behavioral needs.
