Job Summary:

The RN Care Coordinator will work collaboratively with providers, interdisciplinary staff, and clinical associates to ensure seamless care coordination. This includes facilitating effective communication, coordinating services, addressing barriers, and providing education and guidance to patients.

Responsibilities:
  • Collaborate with patients, providers, and interdisciplinary staff to develop and implement care plans
  • Coordinate services, including referrals to specialists, labs, and other healthcare services
  • Provide education and support to patients and their families on chronic disease management, medication adherence, and healthy lifestyle choices
  • Identify and address barriers to care, including social determinants of health and access to resources
  • Document patient information and care coordination activities in the electronic health record
Requirements:
  • Current State Allied Health License required, RN
  • Current Driver's License in good standing and reliable and insured transportation
  • Degree in nursing or other related clinical field
  • Care Management Certification preferred
Preferred Qualifications:
  • Ambulatory and/or care management experience
  • Knowledge of electronic Health Records (EPIC)
  • Excellent communication and interpersonal skills
