$63,500 - $81,300 per year, depending on experience.

Job Description

As a Patient Care Coordinator, you will provide psychoeducation, stress management tools, trauma and grief therapy, treatment for Anxiety and Depression, adjustment issues, suicide risk assessment/intervention, caregiver stress, and help with appealing insurance benefits.

You will work closely with patients, families, and healthcare teams to ensure seamless care coordination. Your attention to detail and excellent communication skills will be essential in this role.
