Key Responsibilities:
  • Conduct comprehensive assessments and develop personalized care plans for patients with complex medical needs
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and optimal patient outcomes
  • Prescribe diagnostics and interventions to prevent unnecessary hospitalizations and promote wellness
  • Stay up-to-date on the latest medical research, technology, and best practices to deliver exceptional care
Requirements:
  • Completion of a Nurse Practitioner or Physician Assistant accredited program
  • Current state RN/NP or PA license
  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)
Preferred Qualifications:
  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
  • Knowledge of long-term care, community resources, and cost-effective alternatives
