Role SummaryThe Gary and Mary West PACE Social Worker plays a vital role in our interdisciplinary team, working closely with participants, their families, and other stakeholders to deliver compassionate, comprehensive health and supportive care. As a Social Worker, you will be responsible for managing a caseload of participants, conducting psychosocial assessments, care planning, counseling, and case management, while collaborating effectively with colleagues and stakeholders to promote our values, team culture, and mission.
Duties and Responsibilities- Participate in the interdisciplinary team and attend morning updates to report changes in participants' baseline status.
- Conduct initial intake meetings with family members and others, and coordinate ongoing family meetings as needed.
- Obtain extensive social history from participants and/or family members upon admission to the program.
- Provide individual and family counseling, develop and lead group counseling and support activities, and provide crisis intervention and advocacy as required.
- Coordinate discharge planning for participants returning home from a hospital or nursing facility.
- Maintain current, written case management records, including ongoing documentation of services provided, reassessment of changing needs, and participant's expressed wishes.
- Act as liaison between participants and other agencies, such as the Department of Aging, Social Security Administration, Medicaid, etc.
- Assist with ongoing financial eligibility for participants, including recertification as needed.
- Participate in participant-related conferences in the community as designated.
- Master's degree in social work required.
- BLS/CPR required.
- Active valid driver's license and proof of auto insurance/Clean driving record.
- Minimum of one (1) year of documented experience in working with a frail or elderly population required.
- Prefer experience in a community-based setting or geriatric program.
- Bilingual in English/Spanish a plus.
