Key Responsibilities- Conduct in-depth psychosocial assessments of Medi-Cal SPD members through home visits and develop personalized care plans in collaboration with the Medi-Cal SPD Case Management Team.
- Provide knowledge, expertise, and resources specific to Medi-Cal SPD members, including information and referrals to community resources and services.
- Collaborate with the Medi-Cal SPD Case Management Team to effectively manage our members' care, prevent re-admissions, and ensure their medical, environmental, and psychosocial needs are met.
- Interact professionally with members, families, case managers, and other healthcare providers to provide support and guidance as needed.
- Document pertinent information completely and accurately in the electronic medical documentation system.
- Master's Degree in Social Work (MSW) or Licensed Clinical Social Worker (LCSW).
- High-level knowledge of Case Management processes and principles.
- Knowledge of Medi-Cal, HMO, Medicare, and Cal MediConnect covered services.
- Excellent communication and organizational skills.
- Ability to work with all levels of management and travel for member appointments.
- Current CA driver's license and car insurance.
