About the Role

We are seeking a highly skilled and experienced Nurse Practitioner to join our team at Central Health Management, Inc. in Lynchburg, Virginia. As a key member of our post-acute care team, you will be responsible for providing high-quality patient care and working closely with our medical staff to ensure the best possible outcomes.

Key Responsibilities
  • Follow up on new patient admissions and adhere to established treatment plans
  • Recertify patient stays and update treatment plans as needed
  • Round on current patients and communicate with medical staff to ensure consistent quality care
  • Update patient charts and prescribe medications as needed
Requirements
  • Nurse Practitioner diploma from an accredited curriculum
  • Board Certification in good standing
  • Active and unencumbered License to practice Nursing in Virginia
  • Active and unencumbered DEA license
  • 3 or more years of experience in bedside patient care, preferably in post-acute care with experience treating co-morbidities
  • Excellent communication skills, diagnostic aptitude, and collaborative/leadership skills
