Job Title: Nurse Practitioner - Post Acute Care Specialist

Job Summary:

Central Health Management, Inc. is seeking an experienced board-certified Nurse Practitioner for a full-time position in post-acute care in the Lynchburg, Virginia area. The ideal candidate will have a strong background in bedside patient care, preferably in post-acute care, with experience treating co-morbidities.

Responsibilities:

  • Follow up on new patient admissions and adhere to the established treatment plan
  • Recertify patient stays and update treatment plans as needed
  • Round on current patients and communicate with medical staff and the physician on-site to ensure consistent quality care
  • Update patient charts and prescribe medications as needed

Requirements:

  • Nurse Practitioner diploma from an accredited Nurse Practitioner curriculum
  • Board Certification in good standing
  • Active and unencumbered License to practice Nursing in Virginia
  • Active and unencumbered DEA license
  • 3 or more years of experience in bedside patient care, preferably in post-acute care

Benefits:

  • Choose your own hours (Monday to Friday)
  • Competitive salary range: $160,000 to $220,000+ annually
