We are seeking a highly skilled and compassionate Director of Social Services to join our team at Fenton Healthcare Center. As a key member of our leadership team, you will be responsible for overseeing the social services department and ensuring that our residents receive the highest level of care and support.
Key Responsibilities- Leadership and Supervision: Supervise and mentor a team of social workers to ensure that they are providing exceptional care and services to our residents.
- Resident Care: Develop and implement care plans that meet the unique needs of each resident, ensuring that they receive the support and services they require to maintain their physical, emotional, and social well-being.
- Communication and Collaboration: Work closely with other departments, including nursing, therapy, and activities, to ensure that our residents receive a comprehensive and coordinated approach to care.
- Documentation and Reporting: Maintain accurate and timely documentation of resident care and services, including progress notes, care plans, and other relevant reports.
- Quality Improvement: Participate in quality improvement initiatives to ensure that our social services department is meeting or exceeding regulatory standards and best practices.
- Education: Master's degree in Social Work (MSW) or related field.
- Experience: Minimum of 2 years of experience in a social services leadership role, preferably in a healthcare setting.
- Licensure: Current Social Work license from the state of Michigan.
