Job Summary

We are seeking a skilled and compassionate Hospice Licensed Practical Nurse (LPN) to join our team. As a Hospice LPN, you will play a vital role in delivering high-quality care to patients and their families, while promoting the FC Compassus LLC philosophy and values.

Key Responsibilities

  • Provide nursing services as directed by the Registered Nurse, including administering medications, treatments, and other care as needed
  • Gather clinical data and provide to RN via documentation and communication to support comprehensive assessment and plan of care management
  • Participate in quality performance improvement projects and clinical record reviews as directed
  • Communicate effectively with RNCM, DCS, and physicians to ensure seamless care coordination
  • Provide education and reinforcement of teaching related to patient care, disease process, symptom management, medications, wound care, and the dying process
  • Perform other duties as assigned

Requirements

  • High school diploma or GED required
  • Minimum of one (1) year of full-time experience as an LPN required
  • Experience in skilled nursing, preferably in hospice, home health care, oncology, or emergency care highly preferred
  • Active and unencumbered Licensed Practical Nurse or Licensed Vocational Nurse in the state(s) of employment required
  • Current CPR certification required

Skills and Abilities

  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
  • Ability to write reports, business correspondence, and procedure manuals
  • Strong written and verbal communication skills
  • Ability to understand, read, write, and speak English
  • Articulates and embraces hospice philosophy
