RN Care Manager

Community Care Hospice LLC is seeking a skilled RN Case Manager to provide exceptional care to our patients.
Key Responsibilities:
- Assess and develop care plans for patients in collaboration with physicians and other healthcare providers.
- Monitor and evaluate patient progress and adjust care plans as needed.
- Provide education and support to patients and their families on disease management, medications, and treatment options.
- Coordinate patient care and communicate with providers, payers, and other stakeholders as needed.
- Advocate for patients and ensure they receive appropriate care and resources.
- Document patient care and maintain accurate records.
- Participate in quality improvement initiatives and ensure compliance with regulatory requirements.
Requirements:
- Active RN license with minimum of 2 years of clinical experience.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of healthcare systems, regulations, and reimbursement processes.
- Proficiency in electronic health records and other healthcare technology.
