Care Manager

About the Role:
WMCHealth is seeking a skilled Care Manager to join our team. As a Care Manager, you will play a critical role in ensuring the delivery of efficient and cost-effective healthcare services to our patients.
Key Responsibilities:
- Maintain familiarity with laws, regulations, and interpretation of utilization review and discharge planning.
- Conduct ongoing quality monitoring, including identification of quality, risk management, and infection control issues.
- Assess patients to identify needs, issues, resources, care goals, and discharge needs.
- Plan patient care collaboration with the healthcare team, patient, and family for the hospital stay.
- Communicate continually with physicians, providing information and interpretation of third-party payer guidelines.
Requirements:
- RN, Graduate of an accredited school of Nursing or BSW/CSW (BSW minimum) or Masters in another health-related field.
- 2 years hospital-based experience, with knowledge of medical/surgical cases.
- One (1) year QA/UR or Case Management preferred.
Preferred Qualifications:
- PRI certification.
- Clinical competency, excellent verbal and written communication skills, and strong organizational skills.
