We are looking for a highly motivated and compassionate Social Worker to join our team at Coffee Regional Medical Center. As a Clinical Social Worker, you will play a vital role in providing holistic care to our patients, addressing their physical, emotional, and social needs.
Your responsibilities will include conducting assessments, developing treatment plans, and providing individual, family, and group counseling sessions. You will also work closely with our interdisciplinary team to coordinate care and ensure seamless transitions between hospitalization and discharge.
To succeed in this role, you must have excellent communication and interpersonal skills, as well as the ability to maintain confidentiality and handle sensitive information with discretion.
