Key Responsibilities- Identify patients with post-acute care needs and develop a comprehensive discharge plan in collaboration with the healthcare team.
- Anticipate and initiate discharge plans for patients with complex needs, working closely with physicians, nurses, and other healthcare providers.
- Review admission assessments and ensure a multidisciplinary plan of care is in place to meet patient needs and desired outcomes.
- Identify system issues that hinder care and participate in developing strategies to remove barriers and facilitate patient progression.
