The estimated salary for this role is $ $70,000 - $90,000 per year, depending on experience and qualifications.

Key Responsibilities:

  • Deliver person-centered care and support services to clients in their homes and community settings.
  • Develop and implement individualized care plans to promote client independence, autonomy, and well-being.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and achieve optimal patient outcomes.
  • Communicate effectively with clients, families, and healthcare professionals to ensure informed decision-making and seamless care transitions.

Requirements:

  • Possesses specialized knowledge and clinical skills necessary to provide high-quality care for individuals with physical disabilities and chronic illnesses.
  • Demonstrates effective communication, problem-solving, and leadership skills.
