We are seeking a highly skilled and dedicated Staff Pharmacist to join our team at Houston Methodist Cypress Hospital. As a Staff Pharmacist, you will play a critical role in providing comprehensive patient care and medication management services to our patients.
Key Responsibilities- Process and verify medication orders, ensuring accuracy and timeliness
- Collaborate with healthcare teams to manage patient drug therapies and monitor for potential side effects and interactions
- Provide patient education and counseling on medication use and management
- Conduct proactive drug therapy reviews and monitoring to ensure optimal patient outcomes
- Participate in quality and safety initiatives to improve patient care and reduce medication errors
- Graduate of an accredited pharmacy program
- Current licensure as a pharmacist in the state of Texas
- Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines
- Strong communication and interpersonal skills, with the ability to work collaboratively with healthcare teams
- Knowledge of comprehensive pharmacy services, including unit-dose drug distribution and intravenous admixture preparation
