Care Coordinator Expert for Home Health Care

BAYADA Home Health Care is seeking a highly skilled and compassionate Care Coordinator Expert to join our team. This full-time position is ideal for individuals who are passionate about delivering exceptional care to our clients and their families.
As a Care Coordinator Expert, you will play a vital role in ensuring the highest quality of care is delivered to our clients. Your primary responsibility will be to coordinate and manage the care of our clients, working closely with our clinical teams to develop and implement personalized care plans. Your exceptional communication and interpersonal skills will enable you to build strong relationships with our clients, their families, and our healthcare professionals.
Key Responsibilities:
Client Care Coordination: You will be responsible for coordinating the care of our clients, including scheduling appointments, managing medication, and communicating with healthcare professionals.
Care Plan Development: You will work with our clinical teams to develop and implement personalized care plans that meet the unique needs of our clients.
Communication and Collaboration: You will be the primary point of contact for our clients and their families, providing exceptional communication and ensuring seamless collaboration with our healthcare professionals.
Requirements:
Education: A four-year college degree is required; prior experience in healthcare, home care, and recruiting is a plus.
Skills and Qualifications: You will need to possess strong interpersonal skills, a demonstrated record of goal achievement, and a proven ability to communicate effectively.
Salary and Benefits: We offer a competitive salary range of $45,000 to $65,000 per year, depending on experience, plus a comprehensive benefits package that includes paid holidays, vacation and sick leave, vision, dental, and medical health plans, employer-paid life insurance, 401(k) with company match, direct deposit, and employee assistance program.
Location: The position is based in the NJ, PA, DE area, with regular travel to Horsham, PA.
Work Schedule: The position is a full-time, 8:30-5pm, Monday to Friday schedule, with the option to work additional hours as needed.
