Key Responsibilities- Assess the psychosocial needs of patients and families, and develop personalized plans to address their concerns and challenges.
- Provide counseling and support to patients and families, helping them cope with stress, anxiety, and other emotional difficulties.
- Collaborate with interdisciplinary teams to ensure comprehensive care and support for patients and families.
- Maintain accurate and detailed clinical records, and participate in quality improvement initiatives to enhance patient care.
- Develop and maintain relationships with community agencies and resources to support patients and families.
- Master's degree in Social Work (MSW) from a Council on Social Work Education (CSWE) accredited program.
- Minimum one year of experience in healthcare, with a preference for hospice experience.
- Current licensure to practice as a Social Worker in the state of operation.
- Excellent communication and organizational skills, with the ability to work effectively in a fast-paced environment.
