Key Responsibilities

  • Perform skilled nursing visits and coordinate client care to ensure optimal patient outcomes.
  • Serve as the primary point of contact for all disciplines involved in patient care, ensuring seamless communication and collaboration.
  • Oversee the frequency of visits for each episode, ensuring timely and effective care.
  • Consult with physicians and office staff as needed to provide detailed updates on patient care and progress.
  • Collect and document accurate OASIS data, ensuring medication profiles are up-to-date and lab values are reported in a timely manner.
  • Attend weekly case conferences and monthly case manager meetings to stay informed and aligned with team goals.

Requirements

  • Current licensure as a Registered Nurse (RN) in the state of employment.
  • A minimum of one year of clinical experience, with a preference for experience in home health or hospice care.
  • Demonstrated knowledge and skill in current nursing practice, with a focus on patient-centered care.
  • A valid state driver's license and automobile liability insurance.
