About the Role
We are seeking a highly skilled Social Care Specialist to join our team at Christ Community Health Services. As a key member of our Integrated Care Team, you will play a crucial role in reducing social barriers to health and facilitating higher levels of care for high-risk mental health patients.
About You
You will have a strong background in social work or a related field, with experience in patient care and social work. Your excellent communication skills and ability to work with diverse populations will enable you to build strong relationships with patients, families, and healthcare teams.
Your Responsibilities
- Respond to identified patient social determinants of health (SDoH) through further assessment and connection to community resources.
- Help support food bank deliveries, transportation to services, insurance sign-ups, and patient engagement as needed.
- Support the psychological well-being of high-risk behavioral health patients on an assigned patient caseload, closely coordinating care with medical providers, BHCs, and other care team members.
- Manage high-risk referral processes and monitor follow-up needs, ensuring ROI is completed for all patients in your caseload for coordinating care with community providers and hospitals.
- Provide backup for BHCs for suicide risk assessments and disposition.
- Receive, coordinate, and track referrals for mental health assessment and treatment with community mental health providers.
- Update patient registries within eHR for patient monitoring and reporting to care teams, clinical team leads, and Clinical Team Lead.
- Function as liaison with mental health and social service partners in the community to coordinate services for patients.
- Work closely with care coordinators through receipt of high-risk referrals and updates of regular care plans.
- Attend regular supervision meetings with the Social Work Lead and required staff meetings.
Benefits
This full-time position offers a competitive salary range of $65,000 - $85,000 per year, depending on experience, plus a comprehensive benefits package, including medical, dental, and vision coverage, 401(k) matching, and paid time off.
