Job Summary

We are seeking a highly motivated and compassionate Health Navigator Specialist to join our team at Equitas Health. This role is responsible for implementing health navigation services, recruiting and engaging clients, conducting screenings and enrollments, assessments, and coordinating referrals to other service providers. The successful candidate will also provide client-centered HIV test counseling services and promote the independence, stability, and health of individuals at high risk of attaining HIV and those living with HIV/AIDS.

Key Responsibilities:
  • Provide high-quality health navigation services to clients, assisting them in accessing medical services, health insurance, and other resources and services to improve health outcomes.
  • Conduct evidence-based HIV prevention interventions in accordance with grant and funder guidelines.
  • Develop and implement community-based presentations on sexual health and the importance of biomedical HIV prevention interventions.
  • Recruit clients through collaboration with DIS, Equitas Health client services, self-referral, physicians, Equitas Health testing sites, health fairs, and other community resources.
  • Plan and conduct ongoing evaluation of program activities to ensure they meet established goals and fidelity.
  • Provide client-centered HIV test counseling services (CTR).
  • Represent the community at local and state HIV prevention planning and/or community groups.
  • BUILD AND MAINTAIN POSITIVE RELATIONSHIPS WITH FUNDERS AND OTHER COMMUNITY ORGANIZATIONS.
  • Collect, analyze, and report data on program activities, ensuring timely and accurate entry into necessary data systems.
  • Complete assessments as needed, focusing on specific barriers and assisting clients in overcoming them.
  • Function as a central access point for financial assistance programs, including PAPI, Gilead Patient Assistance Program, and other programs as appropriate.
  • Provide input and support to the prevention and education department.
  • Stay informed about current HIV/STI trends, developments, and co-factors to transmission, as well as state-of-the-art prevention efforts and strategies.
