**Overview of Prevention Health Navigator Role at Equitas Health

At Equitas Health, our goal is to promote the health and well-being of individuals at high risk of acquiring HIV/AIDS. We are seeking a skilled Prevention Health Navigator to join our team and play a vital role in implementing health navigation services.

As a Prevention Health Navigator, you will be responsible for organizing and conducting various aspects of the program, including outreach and engagement, screening and enrollment, assessment, and coordination of referrals to other service providers. Your expertise will enable you to provide culturally competent health navigation services to clients, assisting them in accessing medical services, health insurance, and other resources necessary to improve their health outcomes.

Key Responsibilities:

  • Provide high-quality health navigation services to clients, ensuring they receive necessary support to achieve optimal health outcomes.
  • Develop and implement evidence-based HIV prevention interventions in accordance with grant and funder guidelines.
  • Engage in community-based presentations on sexual health and the importance of biomedical HIV prevention interventions.
  • Recruit participants through collaborations with local organizations, healthcare providers, and community events.
  • Evaluate the effectiveness of the intervention and ensure fidelity to established goals.
  • Conduct client-centered HIV test counseling services (CTR) and maintain confidentiality.
  • Represent the organization at local and state HIV prevention planning meetings and community groups.
  • Maintain relationships with funders and collaborate with other community organizations.
  • Analyze data on program activities and submit reports as required.
  • Assist clients in completing assessments and documenting barriers to HIV prevention.
  • Support financial assistance programs, such as PAPI and Gilead Patient Assistance Program.

Requirements and Qualifications:

  • Bachelor's degree in a relevant field or equivalent experience.
  • Expertise working with LGBTQIA+ communities and strong understanding of anti-oppression principles.
  • Medical Assistant Certificate is an asset.
  • Strong customer service skills and/or experience in community engagement, advocacy, or organizing.
  • Excellent written and verbal communication skills.
  • Ability to work independently and collaboratively as part of a team.
  • Familiarity with Microsoft Office Products and ability to maintain HIPAA compliance.

**Salary:** The estimated salary for this position is $45,000-$57,000 per annum, commensurate with qualifications and experience.
