Your Role:
As an Oncology Clinical Pharmacist, you will take charge of the medication management for assigned patients. Your responsibilities will include:
- Monitoring and assessing medication therapy for suitability based on patient health status and institutional guidelines.
- Conducting medication reconciliation, monitoring, and providing patient education.
- Engaging in the education of Pharmacy and Medical Residents, along with offering training to healthcare providers as necessary.
- Contributing pharmacy expertise for order sets, formularies, and other pharmacy-related documentation.
Key Responsibilities:
In this role, you will:
- Provide pharmacokinetic consultations and utilize laboratory data to evaluate drug selection and dosing, communicating findings effectively to healthcare practitioners.
- Assess patient responses to drug therapies and document interventions to demonstrate positive patient outcomes.
- Assist in developing clinical pathways, best practices, and guidelines tailored to your patient population.
- Educate staff, students, patients, and other healthcare professionals, with a commitment to continuous learning and improvement.
- Mentor residents and new staff to achieve departmental objectives.
- Ensure comprehensive patient care throughout all transitions, beyond the hospital visit.
- Adapt your approach to meet the specific needs of diverse patient populations.
Physical and Environmental Considerations:
This position may require the ability to exert physical force to move objects and may involve exposure to hazardous medications or waste.
