Key Responsibilities- Provide medication therapy management services to patients, including medication reconciliation, dosing, and monitoring.
- Collaborate with healthcare teams to optimize medication use and improve patient outcomes.
- Develop and implement medication use policies and procedures to ensure safe and effective use of medications.
- Conduct medication use reviews and provide recommendations for improvement.
- Participate in quality improvement initiatives to enhance patient safety and quality of care.
- Doctor of Pharmacy (PharmD) degree from an accredited institution.
- Current licensure as a pharmacist in the state of Louisiana.
- 2+ years of experience in clinical pharmacy practice, preferably in a hospital or health system setting.
- Strong knowledge of medication therapy management principles and practices.
- Excellent communication and interpersonal skills.
- Board certification in pharmacotherapy or a related field.
- Experience with electronic health records and medication management systems.
- Knowledge of quality improvement principles and practices.
