As a Licensed Practical Nurse, you will oversee and manage nursing care and related activities within your assigned unit, ensuring compliance with established policies and regulations.


Key Responsibilities:

  • Adhere to established nursing standards and implement facility policies and procedures.
Provide guidance on policies and procedures to nursing assistants, residents, families, and physicians.

Ensure compliance with all relevant Federal, State, and local regulations within your unit.

  • Receive and review reports from the outgoing nurse, taking necessary follow-up actions.
Communicate essential information to the incoming nurse for continuity of care.

  • Conduct regular resident rounds, supervise C.N.A.s, and report any issues to the nursing supervisor while initiating corrective measures as needed. Monitor changes in resident conditions and communicate with the Nursing Supervisor, physician, and responsible parties.
  • Assign and oversee nursing assistant duties, ensuring quality resident care and providing necessary explanations.

Essential Nursing Skills:
Administer and document medications and treatments as per physician orders and facility policies.

Comply with facility policy and State Law regarding controlled substances.

Manage the admission, discharge, and transfer processes for residents in accordance with facility policies.

  • Assist in data collection as directed by the registered nurse or shift supervisor, collaborating to develop comprehensive care plans and participating in resident care conferences.

Documentation Responsibilities:
Record medications administered or refused, effectiveness of PRN medications, and treatments provided.

Maintain thorough nursing notes to document residents' conditions, changes, and any unusual occurrences as required by regulatory agencies.

Ensure that C.N.A.s complete their documentation accurately.

  • Provide guidance to nursing assistants and recommend disciplinary actions to the Shift Supervisor when necessary.


Contribute to the employee evaluation process by sharing insights on the clinical performance of nursing assistants with the Shift Supervisor.


Additional Clinical Duties:
Check emergency equipment and complete emergency cart checklists.

Identify safety hazards and take immediate corrective actions.


Under the supervision of the registered nurse, notify physicians of any significant changes in residents' conditions, medication errors, or abnormal test results.

Complete shift reports thoroughly.

Practice infection control diligently according to facility policies.

Welcome and mentor new staff members when assigned.

Engage in continuing education relevant to the resident population.

Adhere to all facility safety policies and procedures, and respect residents' rights at all times.

  • Teamwork and Communication Skills
Work collaboratively with residents, nursing staff, physicians, families, and other service providers.

Provide a warm welcome to new residents and their families.

Maintain a professional appearance and demonstrate dependability and punctuality.

Accept assigned duties positively and perform other job-related tasks as directed by Nursing Supervisors or Administration.

Chaperone Responsibilities:

Assist patients during appointments and be present for specific procedures and examinations as required.


Qualifications:

Education:
Completion of an accredited Practical Nursing Education Program.

Experience:
Preferred minimum of one year of experience in rehabilitative, psychiatric, or geriatric nursing.

Licensure:
Current Licensed Practical Nurse license in the State of Connecticut.

Special Skills:
Strong communication and interpersonal skills.


Demonstrate the ability to uphold care standards in accordance with the State of Connecticut Public Health Code and relevant governing bodies.
