This position is responsible for overseeing the care of individuals served assigned to various Levels of Care (LOC), primarily LOC1, LOC2 and LOC3. The Case Manager will work to ensure the continuity of care for individuals served.

  • Key Responsibilities:
  • Attends and participates in all scheduled team meetings.
  • Provides courteous, professional services at all times.
  • Interacts with Center staff, individuals, and community members in a professional manner.
  • Provides Case Management and psychosocial rehabilitation services.
  • Completes community base visits per minimum monthly contact.
  • Provides transportation for individuals as needed.
  • Maintains contact standards as defined in TRR guidelines.
  • Administers clinical assessments and paperwork to determine appropriateness of level of care assignments.
