Job Summary

We are seeking a highly skilled Nurse Practitioner to join our team in Georgetown, SC. As a member of our interdisciplinary team, you will provide high-quality, patient-centered care to our complex patient population.

Key Responsibilities:
  • Conduct comprehensive assessments and develop individualized care plans for patients.
  • Collaborate with healthcare teams to coordinate care and ensure seamless transitions.
  • Provide education and support to patients and their families to promote health and wellness.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.
  • Stay up-to-date with relevant medical and nursing research, technology, and best practices.
Requirements:
  • Current state RN license and NP license or ability to obtain before start date.
  • Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
  • Access to reliable transportation.
Preferred Qualifications:
  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
  • Knowledge of long-term care, community resources, and cost-effective alternatives.
  • Knowledge of CPT, ICD-10, and HCPC codes.
